Osseous Surgery – Bone Grafting to Save Teeth

At Bedrock Dentistry, our osseous surgery with bone grafting helps restore bone lost due to gum disease, protecting your natural teeth. The procedure reshapes and rebuilds the bone structure to support healthy gums and teeth stability. Trust us to preserve your smile with advanced care tailored to your needs.

What is osseous surgery and why might I need it?

Osseous surgery—also called pocket reduction surgery—is a specialized gum treatment that removes bacteria and disease from deep below the gum line while reshaping damaged bone around your teeth. When gum disease advances, it creates pockets between your teeth and gums where bacteria hide and destroy the bone that holds your teeth in place. Left untreated, this bone loss causes teeth to loosen, shift, or eventually fall out.
During osseous surgery, we gently clean infected tissue, smooth irregular bone surfaces, and add bone grafting material where needed to rebuild lost structure and give your teeth a stable foundation again. This procedure stops gum disease from progressing, reduces pocket depth so bacteria can't hide, and creates the healthy conditions your gums need to reattach and heal properly around your teeth.

Who benefits from osseous surgery?

You may be a candidate for osseous surgery if you have moderate to advanced periodontal disease with deep pockets (typically 5mm or deeper), bone loss visible on X-rays, or teeth that feel loose or painful when chewing. Many patients come to us after being told their teeth can't be saved, but osseous surgery with bone grafting often gives your natural teeth a second chance and delays or avoids the need for extractions and implants.

What happens during the procedure?

We numb the treatment area completely so you feel no pain, then carefully fold back the gum tissue to access the roots and bone underneath. After removing infection and tartar, we reshape any uneven bone, apply bone graft material to fill defects, and close the gums snugly around your teeth to promote healing. Most patients return to normal activities within a few days and notice firmer gums and less bleeding within weeks as the bone rebuilds and gums tighten.

What are the post-operative instructions after osseous surgery with bone grafting?

After your osseous surgery with bone grafting at Bedrock Dentistry, following the right care steps protects your healing and ensures the bone graft successfully rebuilds the support around your teeth. Recovery happens in stages, and we'll guide you through each phase so you know exactly what to expect.

First month after surgery: Protect the surgical site

Do not brush or chew on the teeth where surgery was performed for the first month until your stitches are removed. This critical healing window allows the bone graft material to settle into place and the gum tissue to seal properly around your teeth without disruption. Brushing or chewing pressure can dislodge the graft particles, open the stitches, or introduce bacteria that cause infection.
Instead, you'll rinse twice daily with a special antimicrobial rinse and apply a medicated gel we provide to keep the area clean and reduce bacteria without mechanical scrubbing. Continue brushing and flossing your other teeth normally to maintain overall oral hygiene, but carefully avoid the surgical area.
Stick to soft foods that don't require chewing on the treated side—think smoothies, yogurt, mashed potatoes, scrambled eggs, soups, and protein shakes. Avoid hot, spicy, crunchy, or sticky foods that could irritate the gums or pull at the stitches.

One month post-op: Stitch removal and gentle resumption

At your one-month follow-up appointment, we'll remove your stitches and examine how well the soft tissue has healed. Once stitches are out, you can begin brushing the treated teeth very lightly with a soft-bristled toothbrush and start eating gently on that side, gradually reintroducing firmer foods as comfort allows.
Important: Even though the gums look and feel healed on the surface, the bone graft underneath is still maturing and hardening. The soft tissue typically heals within 4–6 weeks, but the bone regeneration process takes much longer.

Four months and beyond: Bone maturation and evaluation

The bone graft material needs at least four months to fully mature and integrate with your natural bone structure. During this time, avoid heavy chewing forces, hard foods, or habits like teeth grinding that could stress the healing bone. Your periodontist will not probe the pockets or perform deep cleaning around the grafted teeth until this maturation period is complete, because early probing can disrupt the delicate new bone formation.
After four months, you'll return for a comprehensive periodontal evaluation where we measure pocket depths, assess bone regeneration with X-rays, and confirm that your teeth have regained stable, healthy support. At that point, you can resume normal eating, regular dental cleanings, and full function of your treated teeth.

Why is following these instructions so important?

Skipping steps or returning to normal brushing and chewing too soon dramatically increases the risk of graft failure, infection, or incomplete bone healing—which means the procedure may not save your teeth as intended. Osseous surgery with bone grafting gives your natural teeth a second chance, but success depends on giving your body the time and protection it needs to rebuild strong, lasting bone support.
If you experience severe pain, swelling that worsens after 48 hours, persistent bleeding, or foul taste or odor from the surgical site, contact our office immediately. We're here to support you through every stage of recovery and answer any questions that come up along the way.
